Voluntary option for undergraduate and graduate students at all Indiana University campuses (Bloomington, Indianapolis, Gary, Richmond, Kokomo, New Albany, and South Bend). - School of Medicine and School of Dentistry Plan
Mandatory plan for IU School of Medicine students, IU School of Dentistry students, and Post-Doctoral Fellows. - Student Academic Appointees (SAAs) and Fellowship Recipients Plan
