OPEN ENROLLMENT 2009 REMINDER
(Full-time Academic and Staff Employees)
Friday, November 14, 2008, is the deadline for:
- Making changes in medical, dental or Personal Accident Insurance (PAI) coverage for 2009. You may:
- Add or drop medical, dental or PAI plan coverage
- Change from one medical plan to another
- Change the dependents covered under your medical and/or dental plan
- Change insurance levels under PAI coverage
New for January 1, 2009: IU HDHP (High Deductible Healthcare Plan) PPO & Medical Savings Plan—this new and unique plan combines comprehensive medical coverage with a health savings account and health risk and biometric assessment features.
- Enrolling in the Tax Saver Benefit (TSB) program for medical and/or dependent care expenses for 2009.
- A new enrollment is required each year for TSB medical and dependent carereimbursement accounts.
- TSB plan participants who have not done so before may elect the IU TSB card option at the Nyhart Web page at https://iutsb.nyhart.com between December 15 and December 31, 2008. Cards for new participants will be mailed in January. Participants who have previously received a TSB card do not need to reapply for a new card.
- Enrolling in the Wellness Health Reimbursement Arrangement (HRA) Plan by completing an affidavit (Section 7 of the Open Enrollment form) stating the employee does not use tobacco and will not during 2009, or that the employee will complete the IU-sponsored Quit for Life tobacco cessation program by June 1, 2009 and a follow-up survey six months from the quit date. Please note that an affidavit is required each year and failure to re-enroll results in forfeiture of any 2008 account balances. (Claims for services received in 2008 must be submitted to Nyhart by April 15, 2009.)
Enrollment may be done online through Benefits Self Service, accessed through OneStart from November 3 through November 14. Online enrollment is required for those electing the new IU HDHP PPO & Medical Savings Plan and optional for other enrollment elections.
