OPEN ENROLLMENT 2007
(Full-time Academic and Staff Employees)

Open enrollment presentations on health care options and the Tax Saver Benefit plan provisions are scheduled:

Wednesday, November 1	Willkie Auditorium, 1st Floor	9am-11am
Monday,  November 6	IMU, Whittenberger Auditorium	9am, 12pm, 2pm
Monday, November 13	Cyclotron, North Large Conf Room	3pm-4:30pm

Friday, November 17, 2006, is the deadline for:

• Making changes in medical, dental, or Personal Accident Insurance (PAI) coverage for 2007.  You may:
  • Add or drop medical, dental or PAI plan coverage
  • Change from one medical plan to another
  • Change the dependents covered under your medical and/or dental plan
  • Change insurance levels under your PAI coverage
• Enrolling in the Tax Saver Benefit (TSB) program for medical and/or dependent care expenses for 2007.
  • A new enrollment form is required each year for TSB medical and dependent care reimbursement accounts.

Return completed enrollment forms to University Human Resource Services, Poplars, Room E165.
