FLU SHOT PROGRAM
From October 22 to November 2, 2007
The annual flu shot clinic sponsored by the University Human Resources Services office and provided by the IU Health Center will be available from October 22 to November 2, 2007, for Academic and Staff employees. Retirees are eligible but must pay $21.25 at the time of service. To take advantage of this preventive medicine program, please call the IU Health Center and make an appointment for your flu shot (812) 855-7688 (choose option one). *You must have your Employee ID number when calling to schedule an appointment.
If you are enrolled in the PPO $900 Deductible, Blue Preferred Primary POS or PPO-Plus health care plans, the flu shots are paid for by Indiana University. PLEASE BRING YOUR EMPLOYEE ID NUMBER AND HEALTH PLAN ID CARD TO THE HEALTH CENTER WHEN YOU COME TO GET YOUR FLU SHOT. There is a $21.25 charge for those not covered by the three plans mentioned above, which must be paid at the time the injection is given.
You will NOT be able to obtain a flu shot if you fail to bring your Employee ID number & Health Plan ID card.
If you are not covered by a health plan and want to receive a flu shot you may pay $21.25 by check (only). No credit or debit cards or cash will be accepted due to the remote location of the clinics.
People at greatest risk for flu:
There are some individuals who are at serious risk if they get the flu and an annual flu shot is recommended:
- anyone over the age of 50;
- people with a long-term health problem such as heart, kidney or lung disease or diabetes;
- people with illnesses or medications which weakens their immune system;
- people in close contact with people in the above risk groups;
- all women who will be pregnant during the flu season (usually November – March).
The IU Health Center strongly recommends that people in these categories be immunized. If you do not fit the aforementioned categories but would like to reduce your risk of being ill from the flu, call today for an appointment.
Call 855-7688 (choose option one) for an appointment with the Immunization Nurse. Individuals who do not keep their appointment or cancel it less than two hours in advance will be assessed $10.00. This charge will be billed to the individual Academic or Staff employee. Other flu shot sites on campus are listed on the IU Health Center web page at www.indiana.edu/~health.October 12, 2007
