About the file formats
HTML: The HTML form can be completed online then printed. Please note, when typing into text boxes only the text that is visible will print. If you plan to input more text than the space available please use the RTF form instead.
PDF (portable document format): PDFs can be opened using the free Adobe Acrobat Reader program. They must be printed, completed by hand and then return as directed. Not recommended for applicants with ADA accessibility issues.
RTF (rich text format): This format can be opened and completed with any word processing program: MS Word, Word Perfect, AppleWorks, etc. The completed form should then be printed and returned as directed.

Excel: If your browser asks what to do with the Excel file when downloading choose either "open with..." MS Excel or "save to disk".
