Last year, Premier Healthcare made a business decision to cease participation in Anthem’s PPO provider network effective November 1, 2012. Unless Premier rejoins Anthem’s PPO network, members of the IU PPO $400 Deductible, IU PPO $900 Deductible, and IU HDHP PPO & HSA plans will continue to receive out-of-network benefits in 2013 for Premier physicians and for laboratory and diagnostic services billed by Premier.

The following information is provided to assist employees in understanding out-of-network benefits and in finding alternative services in-network.

The Blue Retiree plan, a medicare supplement, is based on the Medicare provider network and is not affected by Premier's termination from Anthem's PPO network.
