Subject: PPO Provider Network


Premier Healthcare (formerly IMA) has made a business decision to cease participation in Anthem's PPO provider network effective October 31, 2012. Having received Premier Healthcare's written notice of termination, Anthem is required to notify plan members of the upcoming change in network status. Plan members using Premier providers are beginning to receive such a letter.

If Premier Healthcare does not re-join Anthem's PPO network by November 1, 2012, individuals covered by the IU PPO $400 Deductible, IU PPO $900 Deductible, and IU HDHP & HSA plans will need to consider other in-network providers for the highest level of benefits.
