Changes Across All Medical Plan Options
- The Prescription Benefit Manager for all retail and mail order prescription drugs will change to Medco, Inc. This change is consistent with a state of Indiana requirement for public universities.
HDHP PPO & Health Savings Account (HSA) Plan
- IU contributions to the HSA feature will increase to $1,200 annually for Employee Only and $2,400 for Family
- IRS annual contribution limits will increase to $3,100 for Employee Only and to $6,250 for Family
- Change to Medco will result in an enhanced list of preventive medications with no deductible
IU Health Quality Partners Plan
- Eligibility expanded to residents of Brown, Green, and Howard counties
- Primary care copay will be $30; specialist copay will be $40
- Hospital admission copay will be $400; outpatient facility copay will be $200
- Copays for other services will be 15%
- Emergency room copay will be $150, waived if admitted
- Urgent care copay will be $75
- Prescription drug copays will change to that of the PPO plans
PPO $900 Deductible Plan
- After meeting deductible the copay will be 20%, with no change in Rx copays
- Out-of-pocket maximum for out-of-network benefit will be $3,000 per member with $9,000 per family
- Urgent care copay will be $75
- Emergency room copay will be $150, waived if admitted
PPO Blue Access $400 Plan
- Title of this plan will change to PPO Blue Access $400 Plan
- Deductible will be $400 per member with maximum of $1,200 per family
- Copays will be standardized to 20%
- Out-of-pocket maximum for out-of-network benefit will be $3,000 per member with $9,000 per family
- Urgent care copay will be $75
- Emergency room copay will be $150, waived if admitted

Vision Wear Program
Voluntary Benefits Program
The Vision Wear Program provides benefits and discounts for glasses and contacts with services received from private ophthalmologists and optometrists and retail optical providers. Benefits and discounts are given at the time of service or when a purchase is made.
Current participants will receive a notice from Marsh, the plan administrator, about the opportunity to enroll in coverage during the upcoming Open Enrollment period with an effective date of January 1, 2012.
IU Tobacco-free Program
As an incentive for the non-use of tobacco and to promote a healthy life, the University provides the IU Tobacco-free Program which includes two features.
- Tobacco cessation assistance is fully paid by the University. This assistance covers the cost of employee and spouse/domestic partner participation in Alere Wellbeing's (formerly Free & Clear, Inc.) Quit for Life™ program.
- $25 per month for an employee or spouse ($50 for both) who do not use tobacco will be deducted from the employee's contribution to the medical plan. The employee must sign a Tobacco-free Affidavit. Those who are not tobacco-free at the time of signing the affidavit are eligible for this reduction after notifying the University that they have completed the Quit for Life program.
