Health Risk Assessment
Remove the Guesswork from Personal Health
Indiana University is partnering with Clarian Healthy Results to help increase employees’ awareness and understanding of how to actively improve their health. One of these programs is the HealthMedia® Succeed™ Health Risk Assessment. The assessment takes the guesswork out of finding credible, pertinent health information on the Internet and elsewhere. After completing a 20-25 minute confidential health history and questionnaire, participants receive a report with results of their health status and personalized feedback. Depending on one’s needs, s/he may be invited to participate in programs designed to help lose weight, quit smoking, beat insomnia, care for back pain, and more.

Using Funds in the Tobacco-free Wellness HRA Account
Participants who have a Tobacco-free Wellness (Health Reimbursement Arrangement) account can utilize their funds by paying for services and purchases in one or both of the following ways:
- Pay using the TSB-HRA Card option.
- Pay out-of-pocket and submit claims to Nyhart for reimbursement by check or electronic fund transfer.
Pay using the TSB-HRA Card option
The TSB-HRA Card is a debit-type MasterCard that allows participants to pay for services directly from their account. Participants use the card to pay at the time of service, keeping their receipts. Each time the TSB-HRA Card is used, the participant may receive a request (e-mail or U.S. mail) from Nyhart to substantiate purchases with receipts.
Pay out-of-pocket and submit an HRA claim
To submit a claim online, use the Nyhart web page iutsb.nyhart.com. The employee ID and last 4 digits of the Social Security Number are required to log in the first time.
Paper claim forms are available at a campus Human Resources office or can be printed from www.indiana.edu/~uhrs/pubs/forms/tsbclaim.pdf. Paper claims can be mailed to the address below or faxed to (888) 887-9961.

Account balances and claims
In most cases, unused year-end account balances up to a $240 maximum carry over into the next year. See www.indiana.edu/~uhrs/benefits/hra.html for exceptions.
Employees enrolled in both the Wellness HRA and TSB Plans
Nyhart will reimburse claims using Tax Saver Benefit (TSB) funds first since they do not roll over. Employees can view their HRA and TSB accounts side by side on the Nyhart Web site. The TSB-HRA card may be used for HRA claims as well as TSB plan claims.
FLSA Compliance for PAO and PAU Staff
Switch to Biweekly Paycycle in August 2009
Positions classified as PAO and PAU are considered Professional Staff, with incumbents subject to Fair Labor Standards Act (FLSA) regulations for recording hours worked and overtime pay. A paper-based timesheet is currently used for recording hours worked, including the determination of overtime.
A review of paper timesheets by Internal Audit found significant FLSA compliance issues for PAO and PAU Staff employees. To address the audit findings and to install a more effective system that tracks hours worked and overtime pay, as required by FLSA regulations, PAO and PAU Staff employees will be switched from the University’s monthly payroll cycle to the bi-weekly cycle, effective August 23, 2009. The bi-weekly payroll cycle and the TIME system are specifically designed to track hours worked and overtime, whereas the monthly payroll cycle focuses on “monthly salary” for positions exempt from FLSA regulations.
