PPO Medical Care Plans Provide Information to Stay Well and Healthy
Health and disease management programs are among the services that Anthem provides to employees enrolled in IU-sponsored PPO and POS plans—the IU PPO $900 Deductible Medical Care plan, IU PPO-Plus Medical Care plan, and Blue Preferred Primary POS plan. These programs consist of coordinated health care communications and interventions for individuals with conditions in which patient self-care efforts play a significant role in improving health and preventing complications.
Anthem offers these programs for individuals with asthma, diabetes, congestive heart failure, chronic respiratory diseases (like emphysema), high risk pregnancy, and congestive heart failure. These programs are voluntary and are offered at no additional cost to plan members. The programs provide members with guidelines, reminders, information and encouragement to stay well and healthy. Reminders may also be sent for preventive diagnostic services and immunizations.
Plan members automatically receive educational information from Anthem if their claims indicate they may have a high-risk pregnancy, have diabetes, have suffered a heart attack, or had surgery related to coronary artery disease. Members may also enroll for these services online at Anthem's website.
