2004 PPO Medical Care Plan Booklets Available
The 2004 IU PPO-Plus Medical Care Plan and IU PPO $900 Deductible Medical Care Plan booklets are available on the University Human Resource Services Web site here. Summaries of plan benefits are also available “at-a-glance” on the Web site.
Employees who do not have Web access and need a plan booklet mailed to them should contact the University Human Resource Services’ publications coordinator at 812-855-2985.
The plan booklets reflect the benefit changes for the 2004 plan year.

One comprehensive eye exam, including refraction, is covered each year—the employee pays only the plan copay.

The prescription brand copays have increased to $15 (for brands under $60) and $30 (for brands $60 and over).

Nonsedating antihistamines have been moved to Tier 4, 100 percent copay with plan discounts. Nonsedating antihistamines include Allegra, Zyrtec, and Clarinex.

For the PPO $900 Deductible plan, preventive services are covered with just a copay (no deductible is applied) and the family deductible and copay maximums have increased to a $2,700 family deductible and $3,000 family copay maximum.
The 2004 IU Dental Plan, M-Plan HMO, and Blue Preferred Primary POS booklets are also available at the University Human Resource Services Web site.
