Eligibility

Medical Residents appointed full time and part time but not less than 50% by the IUSM Office of Graduate Medical Education are eligible to receive health benefits, including medical and dental coverage. New Medical Residents will receive a notification email from Human Resource Services with information on how to enroll.

Family Status Change: Enrollment begins on your date of hire, and you cannot make any changes until the next Open Enrollment period. However, if you experience a qualified change in status (e.g., marriage or birth of a child), you can make mid-year changes online by visiting the Benefit Change Connection. Family status changes must be completed within 30 days of the date of the change in status.


Medical Plan

The plan is administered by Anthem and is available to all eligible Residents, regardless of area of residence. When network providers are used, participants receive full benefits. Partial benefits are received from non-network providers, except in the case of an emergency. Network providers include Anthem Blue Access PPO providers in Indiana, Ohio, and Kentucky and BCBS Blue Card providers in other states.

Prescription Drugs (Rx)

This medical plan’s prescription benefit includes medications available on the Anthem Formulary, which may offer potential savings when a physician prescribes formulary medications.

Mental Health and Chemical Dependency

This medical plan offers full benefits from Anthem Behavioral Health network providers. Partial benefits are received from non-network providers. All services must be authorized in advance in order to be covered.

Vision

This medical plan's vision program is Blue View Vision-Exam Plus Option #5.  Anthem has an extensive nationwide network of doctors who provide quality eye care and materials. When you are seen by a participating doctor, your plan covers an eye examination, less any applicable copayment. You may also purchase eyeglass frames, lenses, and contact lenses at a discount.

Dental Plan

Residents and their dependents have a choice between two dental care plans: CIGNA Dental Health Maintenance Organization (DHMO) and CIGNA Preferred Provider Organization (PPO). You will be allowed to enroll in one of these plans at the beginning of your training. If you decide not to enroll, you must wait until the next open enrollment period to receive this benefit. Also, you may change plans only during open enrollment.

DHMO

This program emphasizes diagnostic and preventive services and offers services and benefits through a limited network of participating dental locations. There is no coverage for out-of-network services. With the DHMO there are no deductibles or claim forms. You will receive services at no charge for x-rays, routine cleanings, topical fluoride, oral exams, and local anesthesia. All other procedures are covered at substantial savings to you.

PPO

This is a traditional plan that provides reimbursement for dental services from a network of participating providers, or at a lower benefit, outside the network from any licensed dentist of your choice. You are required to pay the dentist for normal charges and then file a claim for reimbursement. Under this program, you have an annual deductible of $50 for a single plan and $150 for a family plan. There is also an annual benefit maximum amount of $500 per person per plan year.

Optional Plans

Retirement

Optional retirement plans are available to full-time (100% FTE) and part-time (>50% FTE) Residents.

Enrollment or changes in the optional retirement plans can take place through Benefits Self Service at anytime during your residency beginning September 1, 2011.

Tax Saver Benefit (TSB) Plan

The Tax Saver Benefit Plan is available to full-time (100% FTE) Residents only. It allows eligible Indiana University employees to reduce out-of-pocket costs for eligible medical and dependent care expenses by using "tax-exempt" dollars.

Personal Accident Insurance (PAI)

Personal Accident Insurance plan pays benefits in the event of accidental loss of life or severe injuries which result in dismemberment. It is available to full-time (100% FTE) Residents only.
