photo of Ann Baas

Ann Baas, RN, BSN

Ann Baas graduated from Indiana University (1990) and spent more than a decade as a neonatal nurse in the Special Care Nursery at Indiana University Health Bloomington Hospital.

Ann began providing direct pediatric patient care at IU Health Southern Indiana Physicians Southern Indiana Pediatrics beginning in 2001.

In addition to her current role as Care Manager for Indiana University pediatric health plan members, Ann also coordinates the reporting of quality data for the practice.

Dan Rademacher, RN, BSN

Dan Rademacher graduated from Indiana University with a BA in Biology (1991) and a BS in nursing (1994).  Dan started with IU Health Bloomington Hospital as an Emergency Department Technician and served as an RN there for nearly 10 years.

He spent two years as a clinical manager with Unity Physician Group and two years as director of medical/surgical nursing with IU Health Morgan Hospital in Martinsville. Most recently, Dan served as Director of Health Services at the Monroe County Community School Corporation. 

He currently provides care management services for Indiana University’s Clinical Care Services program for patients at participating IU Health Southern Indiana Physicians primary care offices.

Kimberly Nelson, APRN-BC

Kimberly Nelson received a BS in nursing from Ball State University (1986), an MS in nursing from Indiana University (1996) and a post master’s degree from the University of Indianapolis as a family nurse practitioner (1998).

For 15 years, Kimberly worked as an adult nurse practitioner for health care providers in Lafayette and central Indiana, with a six year stint as a lecturer/clinical preceptor for Purdue University’s Adult Nurse Practitioner program.

In 2011, Kimberly began work as an adult nurse practitioner for Premier Healthcare, LLC where she conducts annual wellness visits for Medicare beneficiaries and provides care management services for Indiana University’s Clinical Care Services program.

Jennifer Reiter, PharmD, BCPS

Jennifer Reiter graduated from Purdue University (2008) as a Doctor of Pharmacy. She has seven years of medical teaching experience.

Jennifer started working with IU Health Bloomington in 2008 as a clinical pharmacist. In 2010, she became a clinical pharmacy specialist and began working with IU Health Southern Indiana Physicians, where she currently reviews patient charts for medication-related problems and assists in the management of chronic disease states.
